If you have taken the CDC Concussion Awareness course and have your certificate, email a copy with your name and eAYSO ID number to ColleenDalit@ayso.org and request credit for taking the Concussion training.

All volunteers (coaches, referees, board members, etc) in Chappaqua AYSO are required to take the CDC  Concussion Awareness class offered through aysou.org
